Evaluation of Nutritional Status among the Vulnerable Elderly: Focused on the Recipients of Meals-on-Wheels

Abstract
With an increasing aging population, there is a growing interest in nutrition in later life, which is essentially a crucial part of good health. The purpose of this study was to evaluate the nutritional intake and quality of meals by using the 24-hour recall method, and to identify the association between anemia and nutrition status, food insecurity, and nutrient intake. Subjects were the non-active elderly who were just starting to utilize the meals-on- wheels service in Seoul. The results of prevalence rates for nutritional problems showed as follows: 44.1% for anemia, 12.6% for underweight, 37.0% for obesity, 14.7% for malnutrition diagnosed with a mini nutritional assessment tool (MNA), and 84.8% for food insecurity. The average daily energy intake was 1,374.2 kcal for male participants and 1,131.3 kcal for female participants. The proportion of participants eating less than 75% of the estimated energy requirement was 58.4%, whereas consumption of calcium and vitamin C below the estimated average requirement was 90.4% and 88.8%, respectively, indicating inadequate nutrient intakes. The risk of anemia was associated with lack of vitamin A (odds ratio 5.549), calcium (odds ratio 7.883), and iron (odds ratio 4.470) intakes. This study supported that, in terms of the nutrition perspective, criteria of food insecurity, the MNA tool, underweight, and anemia are useful for determining a public health service beneficiary. In future, it is important to develop the meals-on-wheels system for ensuring adequate levels of nutrient intake among the elderly.
